Please sign you name, attach a photocopy of both sides of your credit card and send the Authorization Form to DTI SERVICES by certified mail or certified express mail services,
such as FEDEX, UPS, or DHL.

You will receive a confirmation email from DTI SERVICES within three days
after we process your request.
* Please do not send Authorization Form by regular email.
* Your credit card number(s) and signature(s) must be legible. You may be asked to re-submit copies of your credit card(s) if they are illegible.
* If you do not own a fax or copier, please go to a local business center for assistance.
* Sender must to pay for the stamp, shipping and other fees.
* Should you have any questions regarding the Credit Card Authorization Form, please contact Customer Support.
All information sent to DTI SERVICES, INC. will be kept confidential and complete discretion will be used when processing your Credit Card Authorization Form.
No third-parties will be given any of your personal credit card information and only the amount agreed upon will be charged.
